How fentanyl came out of a Belgian lab in 1960

Fentanyl was first synthesized in 1960 by Dr. Paul Janssen, the Belgian pharmacologist whose company later became Janssen Pharmaceutica. Janssen was hunting for an opioid that worked faster and more potently than morphine, and he found it: the compound was roughly 50 to 100 times stronger than morphine on a milligram-for-milligram basis. The word “fentanyl” is still a brand-free generic term in the U.S., though it remains a Schedule II controlled substance under federal law.

For context on how steep the potency curve runs, consider epibatidine, a toxin from Ecuadorian poison frogs that is about 200 times more potent than morphine. Fentanyl sits in the same alarming neighborhood of potency, which is why dosing is measured in micrograms, not milligrams, and why a tiny amount of stray powder can be dangerous.

Those fentanyl pictures that circulate online — often blue “M30” pills that look like oxycodone — are worth understanding. Counterfeit pills pressed with fentanyl have driven most of the recent U.S. overdose deaths involving fake prescription drugs, according to DEA reporting.

What was fentanyl originally used for?

From the start, fentanyl was designed for anesthesia and surgical pain. It was approved in the United States in 1968 for intravenous use during operations, where its fast onset and short duration let anesthesiologists control pain tightly. A patch form, Duragesic, arrived in 1990 for chronic severe pain, followed by lollipops and buccal tablets for cancer breakthrough pain.

That history explains the drug’s legitimate niche: hospital settings and carefully managed cancer pain, not routine aches. The same speed that helps in an operating room is what makes illicit use so lethal.

The duration of action, by route

How long fentanyl lasts depends entirely on how it enters the body. This is one of the most commonly misread facts about the drug.

Intravenous fentanyl used in surgery has a duration of roughly 30 to 60 minutes for analgesia, though the drug lingers in fat tissue and can accumulate with repeated dosing. The transdermal patch is the opposite: it takes 12 to 24 hours to reach full effect and keeps working for about 72 hours per patch. A provider typically removes the old patch and applies a new one every three days.

Illicitly made fentanyl is unpredictable. Its effects may last only an hour or two, which is shorter than the opioid reversal drug naloxone in some cases. That mismatch is why emergency responders in many U.S. cities now give a second or third naloxone dose and watch the person for hours.

Why the short window matters in real emergencies

Paramedics in Columbus, Ohio, and Phoenix, Arizona describe the same pattern: a person revived from a fentanyl overdose can slip back into respiratory depression as naloxone wears off before the fentanyl clears. The lesson is observation, not just one dose of reversal spray.

For anyone managing legitimate prescriptions, the practical points are straightforward. Never cut or heat a patch, because that releases the dose too fast. Store patches away from children and pets, and fold used patches sticky-side together before disposal so leftover drug isn’t accessible.

Fentanyl remains a tightly controlled tool for anesthesia and severe cancer pain. Understanding its origin and its route-specific timing helps people read headlines and warnings with more accuracy — and recognize when a pill that looks like a pharmacy product is anything but.
